One service, many partners

Seven local organisations have been awarded £758k by Norwich City Council over three years (2018 – 2021) to address rough sleeping and support people with complex needs in Norwich. The project, entitled Pathways, uses the combined strength of its members to offer a pioneering and collaborative model that has not been seen before.

How We Work

The Pathways principle is that there is ‘no wrong door’ for people to access the help they need. Personalised support is focused around each individual, by qualified and experienced Pathways team members, to encourage a positive outcome for each person.

The Pathways team comprises experienced and qualified specialists, employed by the partner organisations and delivered from hubs around the city.

 

What We Do

 

OUTREACH

Provide an outreach service, speaking to people we find sleeping rough on a daily basis.

ADVISE

Provide advice and assistance to people who are rough sleeping or at risk of rough sleeping.

SUPPORT

Support individuals to maintain their accommodation where this will prevent rough sleeping.

SIGNPOSTING

Signpost and support individuals to address barriers to gaining or maintaining accommodation.

REFERRALS

Make referrals into accommodation that is available and is most suitable to meet a person’s needs.
